Charitable Donation Requests

DONATION REQUEST GUIDELINES


Thank you for your interest in Summers Laboratories, Inc. It is our honor to be able to contribute to the many charitable organizations that serve the United States, and we would be happy to consider requests for charitable donations. However,we ask that you submit a formal request in accordance with the Summers Laboratories, Inc.’s official submission guidelines below.

The following policies and procedures must be used for donation requests:

• All requests must come on organization letterhead.

• All requests must come from a registered charitable organization and include a tax identification number. Requests from individuals or commercial entities are not considered for donations.

• Requests must be submitted at least 8 weeks in advance of the event.

• Donated merchandise may not to be resold.

• Due to the large volume of requests we receive, Summers Laboratories, Inc. has established a policy limiting fundraising assistance to non-profit organizations located

within the United States.

• Organization address, phone number, fax number, e-mail address, name of primary contact and executive director.

• One paragraph description of the organization and its mission statement.

• Concise description of the project. Include project goals, demographics and number of people who will benefit from the project, the timeframe and a brief description of

staff responsible for the project.

• Amount of requested contribution.

• Planned use of the contribution. Include a description of any other support for the project.

Summers Laboratories receives many donation requests every year. We try to fulfill as many of these requests as possible in a timely fashion, but please be aware that
we cannot guarantee that every donation request will be met.
